INSTRUCTIONS: 1. A student may register for an Independent Study or Field Study if: * student is a matriculated (accepted) student in one of Warner's degree or certification programs * study will be completed under the supervision of a Warner or UR faculty member (if student wishes to work with someone other than a faculty member [adjunct or expert in field], then student must select a Warner faculty member to act as the instructor of record and to oversee the study) * student's academic record should be free of incomplete or N grades--all classes should have passing grades, unless advisor gives explicit approval * student completes this form and registers for the course online; this form does not register student 2. Student must submit form by deadlines below (if deadline falls on weekend, use following Monday): * Fall ~ September 15th * Spring ~ February 1st * Summer ~ June 15th 3. Student must follow instructions below to avoid delays in processing: * Complete top portion of form with contact information * Select appropriate course number (ED 491 or 591; ED 492 or 592) * Select semester, year, and enter credit hours * Enter title; if no unique title is entered, default title will appear (i.e., Field Study in Education) * In collaboration with instructor, complete the four sections under "Course Description" * Sign and date the form
