WARNER LEAVE OF ABSENCE POLICY: Students requesting a Leave of Absence (LOA) must complete and submit this Request for Leave of Absence form before the term starts or as soon as an event occurs necessitating this LOA. The student’s request for a PERSONAL LEAVE must include a personal statement, documenting the reasons why a leave is needed, as well as when the student expects to return to active academic status. For all other leaves, the student must submit the following documentation to the Warner Registrar’s Office (LeChase Hall, Room 246) within one month of submitting this Request form: (a) for MEDICAL LEAVE, a signed medical professional’s letter, documenting the nature of the request (without details as to the student’s condition) and assessing when the student can return to active academic status; (b) for MILITARY LEAVE, a copy of student’s orders; (c) for PROFESSIONAL LEAVE, a job supervisor’s letter, documenting the student’s need to focus time and attention on job-related activities and estimating when the student can return to active academic status. Professional and/or personal leaves are granted for a total of two (2) semesters only over the course of student’s degree program*. In the case of medical leaves, the student’s healthcare provider determines the length of the leave. Military leaves can be used as often as needed. LEAVES CANNOT BE GRANTED RETROACTIVELY. Once the student submits this form and appropriate documentation to the Warner School Registrar, she will place the student on inactive status. The student should be aware that: • While a LOA will keep a student enrolled at Warner and will have no impact on Warner tuition awards, etc., it does place the student on inactive status; • THIS STATUS WILL AFFECT THE STUDENT’S ABILITY BOTH TO TAKE OUT LOANS AND/OR DEFER LOAN REPAYMENT; • A student on LOA is not eligible for UR student health insurance; full-time enrollment is required to enroll in UR student health insurance; • Leaves are not allowed in the term immediately preceding completion of the student’s degree program.
